Good morning FHCI families,

As part of a data review, we are sending home an updated copy of your student’s Data Verification Form (DVF) on Tuesday March 19th. This form verifies your child’s personal information as well as your contact and emergency contact preferences. This information will be used as part of our family portal initiative, which will be implemented in the future.

Your student will receive their DVF from their homeroom teacher.  If your student has a spare first period, please have them pick up their DVF in the main office.

If you have had any changes to the information contained on the form such as contacts, address, e-mail, phone numbers etc. please sign and return the form with updates by March 29th. The form must be signed by the family member or caregiver with lawful access to the student and will act as an attestation of the accuracy of the information.

If you have not had any changes to the information contained in the form AND you have returned the original form from September, you do not need to return the form.
